 Student Athlete Pre-Participation Medical Forms

Each student-athlete at South Plains College is REQUIRED to complete pre-participation paperwork PRIOR to beginning any team activities. It is important for student-athletes to read and understand the procedures followed by SPC Athletics. Athletes MAY NOT participate until the paperwork has been completed.
